A Return Merchandise Authorization (RMA) number is required for all equipment maintenance and repair requests.
Please complete the appropriate RMA Request Form below to obtain your RMA number.
Once your form is submitted, instructions and your RMA number will be emailed to you within 1–2 business days.
Submitting multiple forms for the same equipment will create duplicate RMAs. If you do not receive your confirmation email, please check your spam, junk, or other folders before submitting again.

Important Information
Repair and Service Timeframe

Typical turnaround: 10–15 business days after your equipment is received.

Return shipping: 3–5 additional business days.
Loaner Equipment

Loaner machines are available for rent at $100 per week (when in stock).

The rental period begins when the loaner is shipped and ends when it is returned to our office.

Loaner machines must be returned within 3 business days after your repaired machine is delivered.
