Return Merchandise Authorization Request

An RMA number is required for all equipment maintenance and repair requests.

To receive an RMA number, please complete the appropriate RMA Request Form below. After submission, your RMA number and instructions will be emailed to you within 1–2 business days.

Please do not submit multiple forms for the same equipment. This will create duplicate RMAs. If you don’t receive your confirmation email, check your spam or junk folder before submitting again.

Maia Duo, Maia Pro, Julian Duo, Julian, Pulse Pro, Sol Pro, Sol, Semi 10, Spiro, AuraWave, Nova HD or ROC Red Light

 

For the following models:

Maxx, Maia, Maia Duo, Semi 5, Semi 5 Duo, Semi 3, or B2

 

Important Information

Repair & Service Time frame

  • Typical turnaround: is usually 10–15 business days after your equipment is received, times may vary.

  • Ground return shipping is around: 3–5 additional business days.

Loaner Equipment

  • Loaner machines are available for rent at $100 per week (when in stock).

  • The rental period begins when the loaner is shipped and ends when it is returned to our office.

  • Loaner machines must be returned within 3 business days after your repaired machine is delivered.

  • Please contact Support to check availability.
